A: Of course you can but you will need to inform your gynaecologist of your desire. An episiotomy is an incision made between the vaginal opening and the anus in an attempt to prevent extensive vaginal tears. It used to be performed routinely by some gynaecologist but not any more and it will depend very much on your gynaecologist style of practice and your preferrence now.
